Prior to your first visit, we will email you paperwork to make your first visit more efficient. If you are unable to print the paperwork, we ask that you arrive 15 minutes early to fill it out in the office. You will complete a medical history form, sign standard HIPPA forms, a financial agreement form, and usually a standardized outcome measurement concerning your injury. If you want to bring in a medical history or medication list, these can be added to your chart as well. On your first visit, the therapist will take a detailed history, complete a full hands on examination, and develop with a treatment plan with goals for expected outcome. We will also initiate treatment on the first day and most times send you home with a few activities to work on at home. Treatment sessions are on average 45 minutes in duration.
When you come in to Mint PT for your initial evaluation, your physical therapist will start out by conducting a full-body movement assessment to test your strength and flexibility as well as to observe functional movements that you complete on a daily basis. Based on your results, they’ll design an individualized plan of care to address any physical dysfunctions, improve your symptoms, and educate you about your condition. The job of a physical therapist is to provide a patient-centered treatment approach, giving you the knowledge and tools necessary to take an active role in your recovery.
